Open our HealthCheck app, then select the photo tile under Optional Tests.

Proceed with your test. When prompted to add pictures, tap on the    icon to open the camera.

Switch to the front-facing (“selfie”) camera by pressing the    symbol, or have someone else take the picture using the regular back-facing camera setting. Hold the camera steady with your driveline site visible on the screen.

Press    on the screen to take the photo.

Select “Use Photo” on the bottom right of the screen, or tap “Retake” on the bottom left of the screen if you’re not satisfied.

Tap on the    icon to repeat the process and send up to four photos with your test. When finished photographing, tap “OK” on the bottom right corner of the screen and continue with your test submission.

Review the information on the Summary screen and tap “Submit” to send your results to your VAD team.

Log in to our HealthCheck app, but instead of clicking on “Start a HealthCheck,” tap the  ...  (“More”) icon on the bottom right of the page.

Tap “Submit a Photo.”

Tap the     icon to open the camera.
